We agree. We tried to get the editor to put the notes at the bottom of the page and we tried to have the notes numbered sequentially. Splitting the notes by chapter is a pain in the ass when citing a work and it is a disservice to the reader.
We get around the problem (a little) by using a large post-it on the page in the index. We had draqn four or five other maps that did not get in—Asi es la vida. LeRoy
